In-Person

The in-person course is designed to provide a comprehensive overview of POLST and allow learners an opportunity to enhance skills in POLST conversation facilitation. It includes a 2.5-hour online prerequisite course followed by a 5.5-hour in-person course. Content includes:

• Information about Advance Health Care Directives since POLST works best when someone has selected the person who would speak for them if they become unable to make their own medical decisions
• Modules focused on learning and facilitating the POLST Conversation with patients and families
• Techniques and tools to improve facilitation skills
• Role-plays and case studies to promote ability, competence and comfort level when facilitating conversations with patients/residents and their families and communicating with their families
• Teaching materials which attendees can use across all health care settings

Please Note: The completion of the online prerequisite course is required prior to attending the in-person session.

Virtual

The virtual course is designed to provide an abbreviated overview of POLST and allows learners an opportunity to enhance skills in POLST conversation facilitation. It includes a 2.5-hour online prerequisite module followed by the 2.5-hour online virutal course. Content includes:

• Information about Advance Health Care Directives since POLST works best when someone has selected the person who would speak for them if they become unable to make their own medical decisions
• Techniques and tools to improve facilitation skills
• Role-plays activities to promote ability, competence and comfort level when facilitating conversations with patients/residents and their families and communicating with their families
